Changing Lives Health and Well-Being service provides professional support to people in the West End of Newcastle whose daily lives are affected by a long-term health condition. We aim to help people take control of their health condition, rather than be controlled by it to lead a more positive and happier life.

The Ways to Wellness is a pioneering, not-for-profit organisation that supports people living with long-term health conditions in the west of Newcastle. The programme is a partnership delivered by Changing Lives, Healthworks, Mental Health Concern and First Clinical. The programme provides non-medical support to people with long-term health conditions through the guidance of a specially trained Link Worker. Link Workers provide one-to-one advice for people who are referred after talking to their GP. Link Workers support people to overcome their current barriers to improve long-term health conditions.

It can be difficult for people with long term health conditions to always feel in control of symptoms.

Long-term health conditions include:

  • Chronic breathing difficulties (COPD) and asthma
  • Diabetes
  • Heart Disease
  • Epilepsy
  • Thinning of the bones
  • Any of the above with depression and anxiety

Through the Ways to Wellness programme and with the one-to-one support from a Link Worker, we can help people to:

  • Manage lifestyle changes
  • Develop a realistic action plan to build skills, knowledge and confidence
  • Build confidence to make positive choices
  • Improve their ability to talk to healthcare professionals about managing health conditions
  • Understand the information given by healthcare professionals
