About the job

As a GAP MAP Health Navigator, you will work as a member of the GAP MAP Project, delivering an assertive outreach service working to support individuals facing social exclusion and sexual exploitation and abuse.

You will work collaboratively to achieve the effective delivery of GAP MAP Theory of Change model to clients living in the Gateshead area. Interacting with men and women, offering emotional and practical support, leading to the stabilisation of their circumstances in order to access appropriate services and support.
